The Nelson House Medicine Lodge is hiring a full-time permanent Intake & Aftercare Specialist to play a pivotal role in fostering strong relationships with referral agents, funders, and efficiently coordinating the intake process. Reporting to the Senior Counsellor, this position involves screening applications for the 8-week co-ed adult residential relatives in recovery treatment program and providing follow-up services. This ensures continuity of care for relatives in recovery who have completed or left the program. Understanding and respect for Indigenous Trauma Informed Practices & Indigenous Gender Informed Practices are foundational to culturally competent and effective care and is an expectation consistent with Best Practices at the Lodge.
The Nelson House Medicine Lodge works with adults with high levels of mental health and substance use disorders who may have been exposed to significant serious or long-lasting trauma. Staff should be aware of the inherent risks of working with this at-risk population we serve as well as other risks that may occur with new admissions.
POSTION SUMMARY:
Intake Duties: Manage the screening, intake, and orientation processes, which are critical components of the NHML in-patient treatment program. Process all applications and enter data into the Addictions Management Information System (AMIS). database. Ensure all documentation is completed and attached before submission to the Screening Committee for review. Facilitate pre-treatment information sessions for relatives preparing to enter the residential treatment program
Aftercare Duties: Provide follow-up support for relatives in recovery who have completed the 8-week program, focusing on their ongoing recovery and continuity of care. Support relatives in recovery with the goals identified in their after-care plans. Facilitate information sessions on Relapse Prevention, Life Skills, and Harm Reduction Strategies. Provide support for relatives in recovery to secure safe and stable housing as needed. Work in collaboration with CFS and Justice to support/advocate. Connect relatives in recovery with training and educational opportunities.
QUALIFICATIONS: Must possess a degree in Social Work or certificate in Counselling, and written skills; be professional in their interaction with our relatives, staff, and public; have knowledge and abilities with human relation skills and telephone etiquette; have a strong consistent work ethic and ability to maintain confidentiality. All applicants must provide a Criminal Record and Vulnerable Persons Check and must sign an Oath of Confidentiality before date of hire.
Must have experience with computers and software programs to assist in creating and maintaining Word and Excel documents; have ability to work with general office equipment; possess strong office and technical skills; have good time management and communication skills and is well-mannered and thoughtful is considered an asset to the business image of an organization.
EDUCATION AND EXPERIENCE REQUIRED: Certificate from a recognized business college or a high school diploma or a combination of training and work experience is required to be able to carry out their job tasks and duties effectively and efficiently.
