POSITION: NCN Medical Receiving Home Patient Navigator

The NCN Medical Receiving Home Patient Navigator is responsible for ensuring the clients of the NCN MRH are transported from the airport/bus depot upon their arrival, to and from their medical appointment, and returned to the airport/bus depot for their departure in a safe and timely manner.

RESPONSIBILITIES

  • Assist in coordinating daily patient transportation schedules for arrival, departure, and medical appointments
  • Keep records of patient transportation
  • Assist patients with disabilities board and exit the vehicle
  • Keep vehicle log records such as mileage tracking, maintenance schedule, and incidents
  • Assist with deliveries such as food or equipment necessary for the operation of the NCN Medical Receiving Home.
  • Keep the vehicle clean
  • Assist in any other duties as directed by the NCN Medical Receiving Home Manager

REQUIREMENTS

  • Class 4 License
  • 2 years’ experience in Patient Navigation
  • high school diploma
  • Workplace Hazardous Material Information System
  • Strong records keeping, organizational and ability to work unsupervised
  • CPR certificate
  • ability to work in a team environment
  • ability to speak Cree is an asset
